I’m a Cash-Based, Out-of-Network Physical Therapy Practice

At Beyond Limits Physical Therapy, I do things a little differently.

I am a cash-based, out-of-network physical therapy practice, which means I do not bill insurance companies directly and I am not an in-network provider with insurance plans.

This allows me to spend our time together focused on you, your body, and finding the underlying cause of what is keeping you from feeling and moving your best—rather than on insurance rules, visit limits, authorizations, or what an insurance company decides is covered.

My goal is simple:
Figure out what is really going on and help you get back to doing the things you love.

What About Insurance?

If you have out-of-network physical therapy benefits, you may be able to seek reimbursement from your insurance company for some or all of your care.

After your payment, I can provide you with a Superbill documenting your visit and the services provided. You can then submit that Superbill directly to your insurance company if you choose to request reimbursement.

A Superbill is not a bill to your insurance company from me, and I do not submit insurance claims or handle the reimbursement process for you.

Whether your insurance company reimburses you—and how much—is determined entirely by your individual insurance plan and your out-of-network benefits.

Because every plan is different, I recommend contacting your insurance company before your first appointment and asking specifically about your out-of-network physical therapy benefits.

Questions to Ask Your Insurance Company

• Do I have out-of-network physical therapy benefits?
• What is my out-of-network deductible?
• What percentage of the allowed amount is reimbursed after my deductible?
• Is there a limit on physical therapy visits?
• Do I need a referral or authorization?
• Do I need to use specific diagnosis or procedure codes?
• How do I submit a Superbill for reimbursement?

What About Medicare?

If you have Medicare, please let me know before scheduling your first appointment.

Beyond Limits Physical Therapy is not a Medicare provider, and Medicare has specific rules regarding physical therapy services and payment. Medicare patients should not assume that a Superbill can be submitted to Medicare for reimbursement.

If you have a Medicare Advantage plan or another insurance plan that provides coverage in addition to Medicare, your situation may be different. Coverage and reimbursement depend on the specific plan.

I will discuss the payment arrangements with you before treatment so you understand what to expect.

Why I Choose to Work Outside the Insurance Model

I have practiced physical therapy for more than 40 years, and I have found that the best care often requires looking beyond the place that hurts.

At Beyond Limits, our work may involve Integrative Manual Therapy, biomechanics, movement, muscle tone, structural relationships, running mechanics, and identifying factors that may be contributing to a problem somewhere else in the body.

That kind of individualized care does not always fit neatly into the traditional insurance model.

Working outside of insurance allows me to spend our time together doing what I believe is most important:

Listening.
Examining.
Finding the root cause.
And treating you as an individual.

There are no insurance-company visit limits determining how much time we can spend together, and there is no insurance company deciding what approach I can or cannot use.

What You Pay

My current rate is $200 per hour of treatment.

Payment is made directly to Beyond Limits Physical Therapy at the time of service.

If you have out-of-network benefits, I can provide a Superbill for you to submit to your insurance company. Reimbursement is not guaranteed and is determined by your insurance plan.

If your insurance does not provide out-of-network reimbursement, you are still welcome to work with me on a private-pay basis.
